Application Forms

First, on your application form, we ask that you agree to the following:

1. SpiritCare's Standards of Ethical Conduct for Employees and Volunteers
2. California Patients Bill of Rights (Regulatory), Title 22, California Code of Regulations Section 72527

Then we ask you about your interests on the form that matches your interests:

1. Application form for a Faith Leader position
2. Application form for a Volunteer position
3. Application form for a Board of Directors position

Your form will be submitted to our spiritcareministry.org email system and sent to the appropriate team leader for review.  If you have any questions about applying or your submission please fill in the form below.
